1. Call the office to see if our office is a right fit for you

2. Once accepted complete these new patient paperwork and medical release form

• Medical Records Release Form
• New Patient Registration
• Authorization of the use of PHI (click for instruction for filling out this form)
• Privacy Policy Form

3. Email that paperwork to [email protected]

We will call to schedule your child's first appointment once we receive records from the previous physician.
